چکیده

Defibrillation success depends upon the amount and distribution of intracardiac current (IC), because only a portion of total current flow between the paddles actually traverses the heart. We evaluated two determinants of IC: paddle size and shock energy. Studies were performed in 11 anesthetized dogs using a damped-sine wave form defibrillator. Intramyocardial electrode pairs were implanted subepicardially to subendocardially across the left and right ventricular free walls and in the interventricular septum. IC was quantitated by oscilloscopic measurements of the electric potential gradient (V/m) developed across the implanted electrodes during synchronized shocks of constant energy. External defibrillating paddle electrodes ("paddles") were applied to the left and right lateral chest walls. An initial group of six dogs was given shocks with 8.5-cm-diameter or 13-cm-diameter paddles to assess the effect of standard vs large paddles on IC. Mean IC values were substantially greater using 13-cm paddles (p < 0.01, paired t test). We then studied a second group of five dogs to determine more precisely the paddle size that produced the highest IC. A series of constant energy shocks using paddles of various sizes (8.5, 10, 12, 14, 16 cm in diameter) were given. In each dog,

BRIEF REVIEW Determinants of successful defibrillation

Ventricular defibrillation by electric counter shock is the definitive treatment for cardiac arrest due to ventricular fibrillation (VF). Ventricular defibrillation is the fundamental cornerstone to a successful outcome. As most deaths from VF occur out of hospital, often in patients without pre-existent recognised cardiac disease, the major public health issue remains the widespread applicatio...

Emergency intracardiac defibrillation for refractory ventricular fibrillation during routine electrophysiologic study.

Ventricular fibrillation refractory to cardiopulmonary resuscitation including multiple transthoracic defibrillations occurred in four patients during 1,215 consecutive ventricular tachycardia induction studies. A technique of emergency intracardiac defibrillation for management of refractory ventricular fibrillation is described. In four patients, stable monomorphic ventricular tachycardia (32...

Transthoracic Resistance in Human Defibrillation

Successful defibrillation depends on delivery of adequate electrical current to the heart; one of the major determinants of current flow is transthoracic resistance (TTR). To study the factors influencing ITR, we prospectively collected data from 44 patients undergoing emergency defibrillation. Shocks of94-450 J delivered energy were administered from specially calibrated Datascope defibrillato...

A Prospective, Randomized Study

Background Recent development of a prototype singlelead unipolar transvenous defibrillator offers the possibility of device implantation with the ease of a permanent pacemaker. Lowering defibrillation energy requirements would allow for a further reduction in defibrillator generator size and enhance the feasibility of pacemaker-like placement. However, if achieving a lower defibrillation energy...

Intracardiac Infection at the Tip of Hemodialysis Catheter: Life threatening Morbidity

Endocarditis and intracardiac infection have been increased recently especially in dialysis dependent renal failure patients. This is usually intractable infection to broad spectrum Antibiotic therapy and in most cases surgical intervention was necessary. We have presented 45 years old man with intracardiac infection at the tip of dialysis catheter that after catheter removal intracardiac infec...
